To transfer your medical records from another practice to Huon Doctors, please print and complete the Transfer of Medical Records form below and email it to manager@huondoctorssurgery.com.au or bring it to your next appointment.

For general enquiries about the services we provide at Huon Doctors, please fill in the Contact form, and we’ll be in touch within two business days. To protect your privacy, you should never include personal information such as your birth date, address, Medicare card number, etc.

Please note: This form should not be used for appointment cancellations. Appointments that were booked online can be cancelled through the booking platform, or by calling our clinic on (03) 6264 1666.
